A5. Alternate firmware is Pseudo Saturn Kai, and as it allows to boot from CD-R it's convenient to test the application in "release mode".

save_id is an extra feature of Gamer's Cartridge to revision history of saves (for example, to recover a save that was deleted or overwritten), so in normal usage setting it always to zero should be fine.
I don't think that there is a game requiring correct metadata to work, but there's maybe a weird case of game using language field to store save data (Bug IIRC, but I may be wrong) and generally speaking it's better to keep metadata as for example comment of a save may give some information...
Legally speaking, only uploading a patch of what is modified from original contents is allowed.
But since that there are folks selling physical copies of translated games (for Ninpen Manmaru and many other other games too), I personally consider uploading a pre-patched disc image as a "bad but...

Regarding the 256KB limit, it is written in a SEGA technical doc (I don't remember which one) and AFAIK there's no game reaching such upper limit, but...
With Tamagotchi Park save, I did a "raw" dump of first 1MB from CS-1 space, and that 1MB ... fitted into internal memory (32KB) after compression. Because Tamagotchi Park save data is full of 00h, that's a champion result, but in typical case it's not negligible.

Good luck in adding compression. Basically anything better than RLE should be fine, so you can for example use bzip2 because it was added in Rockin'-B Save Data Manager, hence shall be easy to merge into SGEX.
