It is very easy to hack the graphics of smaller games like Gameboy. HK dealers will put ANY picture on the box, but you can't return it as long as they made a reasonable effort to make it somehow the title stated on the box.
That cart is probably just a hack of the INTRO title... and the game is probably SuperMarioBros. The box art is irrelevant.
